 Office of the Registrar

​​The Office of the Registrar is committed to providing exemplary service to students, graduates, and faculty. In addition, the Office of the Registrar strives to provide accurate information and efficient services to all KHSC-KansasCOM constituencies. 

The Office of the Registrar protects the accuracy and security of students’ records while also serving as an information resource for students, graduates, as well as faculty and staff. Additionally, the office provides support for students in years three and four of medical as they are applying for clinical rotations and residency.

 Academic Records Request

Requests for academic records will be processed by the Office of Registrar in the order they are received. The Office of the Registrar makes every effort to respond to requests in a timely manner. Requests are typically processed within five (5) business days from receipt. Requests for multiple transcripts, several different documents, or requests during peak seasons of operation may require additional processing time. Information which is required to meet a third-party deadline should be requested with at least two weeks lead time to ensure expedient delivery and appropriate recording.

Please use the Academic Records Request form in the KansasCOM Reporting and Services Request System

For an Academic Records Request, you can select the following:

  • Enrollment Verification form or letter
  • Degree Verification form or letter
  • MSPE: also known as Dean's Letter
  • Letter of Good Standing
  • Other

 Step 1:  Click the link:  KansasCOM Reporting and Services Request System to access the form.

Step 2:  Go to Report Type and choose the Academic Records Request from the dropdown.



updates to personal information

If you require any changes to your personal information, you can submit a Change Request in the Student Gateway.  You will click on the My Personal Info  box, and update any field that has an open box where you can make changes.

If you require a Name Change, a Date of Birth correction, add or correct SSN, add or update Driver's License, please use the KansasCOM Reporting and Services Request System.

Step 1:  Click the link:  KansasCOM Reporting and Services Request System to access the form.

Step 2:  Go to Report Type and choose the Personal Information Update form from the dropdown.

In the Details section, please summarize the request.  This is a required section, and you cannot submit the form without completing it.

Step 3:  Complete and submit the request.


Leave of absence

If you require a Leave of Absence, please reach out to the Associate Dean of Student Affairs and Services, Dr. Rick Winslow for guidance regarding the implications of a leave on your academic pursuits at KansasCOM. 

To submit a request for a Leave of Absence, please use the KansasCOM Reporting and Services Request System.

​Step 1:  Click the link:  KansasCOM Reporting and Services Request System to access the form.

Step 2:  Go to Report Type and choose the Leave of Absence form from the dropdown.

In the Details section, provide a detailed request of your request for a leave.  This is a required section, and you cannot submit the form without completing it.

Step 3:  Complete and submit the request.

Once the form is submitted, the Associate Dean of Student Affairs and Services will reach out to coordinate the checkout process.


Student withdrawal

If you plan to withdraw from KansasCOM, please reach out to the Associate Dean of Student Affairs and Services, Dr. Rick Winslow for guidance regarding the implications of your withdrawal.

To submit a Student Withdrawal, please use the KansasCOM Reporting and Services Request System.

​Step 1:  Click the link:  KansasCOM Reporting and Services Request System to access the form.

Step 2:  Go to Report Type and choose the Leave of Absence form from the dropdown.

In the Details section, provide a detailed summary for your withdrawal.  This is a required section, and you cannot submit the form without completing it.

Step 3:  Complete and submit the request.

Once the form is submitted, the Associate Dean of Student Affairs and Services will reach out to coordinate the checkout process.
